Discussion Evaluation Form

Carefully read and follow instructions at the top of the form.

Focus responses on the indicated topics (e.g., you vs. the class, strengths vs. weaknesses, understanding vs. talking). Students have a tendency to fill the entire form with comments about themselves. The last section is about the class, not about the person filling out the form.

Pledge the form. Submit it correctly.

Each week’s evaluation is due by 10 p.m. that Friday. Late forms are NOT accepted.

Evaluations are required, not optional.

The instructor provides feedback on each form; individual forms (like individual discussions) are not graded. Evaluations contribute to the mid-semester and end-of-course discussion grades.

Sample Paper

As this former student’s paper (used with her permission) demonstrates, it is possible to present the central argument of a book (and thus a much shorter article) in one page. The keys are understanding the article and keeping focus.

  2.  Fill in only the boxed information at the top of the page; the instructor fills in the rest.
  3. Be sure that this sheet is page 1 of your rewrite; your essay is page 2. (Just cut and paste your paper to the end of the sheet.)
  4. Please adjust the grade sheet if your use of it changes the spacing. The info should fit on one page.
